Molecular biology has rapidly advanced since the discovery of the basic flow of information in life, from DNA to RNA to proteins. While there are several important and interesting exceptions to this general flow of information, the importance of these biological macromolecules in dictating the phenotypic nature of living creatures in health and disease is paramount. In the last one and a half decades, and particularly after the completion of the Human Genome Project, there has been an explosion of technologies that allow the broad characterization of these macromolecules in physiology, and the perturbations to these macromolecules that occur in diseases such as cancer. In this volume, we will explore the modern approaches used to characterize these macromolecules in an unbiased, systematic way. Such technologies are rapidly advancing our knowledge of the coordinated and complicated changes that occur during carcinogenesis, and are providing vital information that, when correctly interpreted by biostatistical/bioinformatics analyses, can be exploited for the prevention, diagnosis, and treatment of human cancers. The purpose of this volume is to provide an overview of modern molecular biological approaches to unbiased discovery in cancer research. Advances in molecular biology allowing unbiased analysis of changes in cancer initiation and progression will be overviewed. These include the strategies employed in modern genomics, gene expression analysis, and proteomics.

LUNG Historically, lung carcinoma is more common in men than women. There is a localized cluster of cutaneous non-specific nodules, most often on the anterior chest or abdomen. The diagnosis of lung cancer usually precedes the diagnosis of metastasis to the skin. COLON AND RECTAL Colon and rectal carcinoma is the second most common source of skin metastases in both genders. It usually presents late in the disease course. The abdomen and perineum are the most common sites. Less commonly, it may present as inflammatory metastatic carcinoma of the inguinal folds or as a chronic cutaneous fistula. MELANOMA Melanoma is the third most common cause of metastasis to the skin. Melanoma typically invades the liver, lungs and brain but may also show up at distant cutaneous sites. Skin is the most common primary site of melanoma, followed by ocular and mucosal sites. There are cases of systemic or metastatic melanoma without a primary cutaneous melanoma. Melanoma that is metastatic to the skin typically appears as small 2-5 mm blue papules resembling blue berries. Hundreds of lesions may be present in advanced cases. The Endoscopy Evolution

The evolution of endoscopes has been driven by doctors who have always dreams of bringing better care to patients and by engineers who made efforts to make these dreams come true. It has also been supported by new technological developments and materials that are immediately incorporated into product design in order to accelerate the evolution by leaps and bounds. A new material called glassfiber, developed in the United States in the 1960s, attracted a great deal of attention in various industrial sectors. The developers of endoscopes were among the first ones to turn to glassfiber. Basil Hirshowitz and his associates, for instance, used glassfiber in their endoscopes to take advantage of the glassfiber's characteristic of transmitting light from one end to the other even when it is bent. Their endoscopes allowed direct observation of the interior of a stomach. 1964 when the first gastrocamera with a fiberscope - the much-anticipated camera with an "eye" -was invented. After all these years, doctors became capable of performing real-time observation of the stomach interior for the first time. The era of gastrocameras came to an end around 1975, when they were completely replaced by fiberscopes. Moreover, endoscopes found wider applications for examinations of other body parts including the esophagus, duodenum, large intestine, bronchus and gallbladder. Oleh Haluszka, MD, has joined Temple University Hospital as Section Chief of Gastroenterology. Dr. Haluszka has focused on developing advanced endoscopic procedures that measurably improve the experiences and outcomes of his patients. He is expert in using interventional endoscopy to manage patients with GI bleeding, GI cancer and precancer, and diseases of the pancreas, bile ducts, liver, and gallbladder. His other specialties involve endoscopic ultrasound and placement of stents in blocked bile ducts or other GI passages. At Temple, Dr. Haluszka will continue to focus on therapeutic endoscopy—especially in managing GI malignancies, treating disorders of the small intestine, and collaborating in new types of bariatric and metabolic procedures. Despite our advances in the understanding of the biology and molecular genetics of cancer, as well as the availability of an increasing array of effective therapies, cancer treatment today and for the foreseeable future will include the traditional modalities of surgery, radiation therapy and chemotherapy. Myelosuppressive agents with their potential hematopoietic toxicities remain the mainstay of systemic treatment for both metastatic and early stage cancer. The complications of cancer chemotherapy have serious impact on a patient’s well being and overall quality of life. Fortunately, advances in cancer treatment have been accompanied by equally impressive progress in the availability of a wide array of supportive care modalities which have greatly enhanced the ability of oncologists to minimize the impact of cancer and its treatment on patient quality of life as well improve delivery of potentially curative cancer treatment.

Despite the increasing complexity of modern cancer treatment, it is the obligation of the oncologist as well as the entire cancer care team to be certain that cancer patients receive the optimal supportive care available for their disease and its treatment. Among the most serious and potentially life threatening toxicities of cancer treatment are the hematologic toxicities accompanying myelosuppression including anemia and associated asthenia and fatigue, neutropenia and fever associated with infection in the immunocompromised patient and thrombocytopenia and accompanying risk of bleeding. Special supportive care needs arise in the very elderly care patient that may tax the ability of even the most skilled clinician. Despite the considerable progress that has been made with more effective and safer treatment strategies, myelosuppressive chemotherapy will remain the mainstay of systemic treatment for cancer for the foreseeable future. While considerable progress has occurred, better methods and broader application of supportive care measures are needed to reduce the symptomatic effects of cancer and the associated toxicities associated with cancer treatment.

No area of cancer supportive care better illustrates the progress that has resulted from advances in our understanding of cellular and molecular biology, genetic engineering and the development of more effective yet often more toxic cancer treatments than that of the hematopoietic growth factors. This volume will review and integrate the major advances in our understanding of the underlying molecular biology and pharmacology of these agents along with the results of well designed and executed randomized controlled trials of the erythroid stimulating agents, the myeloid growth factors and the new thrombopoietic agents each addressing a major threat associated with bone marrow suppression accompanying cancer treatment. The current clinical utilization of these agents is based on numerous randomized controlled trials and meta-analyses along with evidence-based clinical practice guidelines developed by professional societies guiding their appropriate and cost-effective use in clinical care.

Colostomy

A colostomy is, depending on circumstances, a reversible or irreversible surgical procedure in which a stoma is formed by drawing the healthy end of the large intestine or colon through an incision in the anterior abdominal wall and suturing it into place. This opening, in conjunction with the attached stoma appliance, provides an alternative channel for feces to leave the body. Indications: There are many reasons for this procedure. Some common reasons are: A section of the colon has been removed, eg due to colon cancer requiring a total mesorectal excision, diverticulitis, injury, etc., so that it is no longer possible for feces to exit via the anus. A portion of the colon (or large intestine) has been operated upon and needs to be 'rested' until it is healed. In this case, the colostomy is often temporary and is usually reversed at a later date, leaving the patient with a small scar in place of the stoma. Children undergoing surgery for extensive pelvic tumors commonly are given a colostomy in preparation for surgery to remove the tumor, followed by reversal of the colostomy.
